Membership in MLHPP's tiered network is non-exclusive and allows clinicians to participate in other clinically integrated networks. There are no requirements for equity contributions or fees to join. MLHPP prides itself as a high performing, physician-led, and shared leadership organization.

MLHPP participation criteria is as follows:

Level I Criteria

  • Use of an EMR for clinical documentation
  • MLHS medical staff membership
  • Use of Epic Secure Chat or cell phone number listed in MLH Physician to Physician Communication app
  • Specialty/multispecialty practices insert EMR after visit summary language encouraging PCP visit and provide to patients
  • Annually attend 1 value based care educational program/event

Level II Criteria

  • Meet level 1 criteria
  • DVACO conditions of participation
